- Supports partner accountability to the Care Coordination (CC) model and to the CC team, including the team's vision, mission, values and guiding principles.

- Uses care coordination performance measures and demographic data in evaluation and continuous improvement, including ensuring efficient and effective operations, and identifying gaps in the continuum of available local resources.

- Conducts de-identified case analysis.

- Listens for and acts on opportunities to support resources and service capacity-building.

- Discusses and plans for sustainability of care coordination.

- Addresses impacts of local agency staff/leadership tenure and turnover.

- Strengthens community-level partnerships, including gathering input from and serving as a liaison to community representatives and organizations.

- Identifies and supports resolution of barriers to service provision.

- Other functions as determined by the Advisory Council.
