Obtain and record VS on 10-15 patients per shift q4 hours including orthostatic BP q shift
Escalate abnormal VS and topics of concern to primary RN including acute changes
Maintained/adhered to isolation precautions
Patient mobility implementation and q2 turn bed rest protocols Documentation of intake and output as and daily weights
Managed intake protocol on new admits to floor with assist of RN Elimination and hygiene assistance
Perineal care including internal foley catheter care and external male and female catheter care
Adherence to hospital protocols and policy amid high acuity environment
Effective and therapeutic communication with patient and family
MedSurg and Advanced MedSurg rotation completed in the following units: Medical Telemetry, Surgical Acute, Cardiac Surgical ICU, and Oncology. Pediatrics and OB rotation in progress
Parenteral and non-parenteral medication administration under observation
Head to toe and focused assessments conducted and documented
Charted patient VS, meals, fluids, output, and activities of daily care Maintained patient care plan and communicated with inter-professional care team to promote continuity of care
Patient teaching on medication administration, adherence, disease management, long term care, and other factors pertaining to individual patient needs
Monitored for s/sx of seizures and implemented emergency care G-tube feeding and monitoring to achieve nutritional objectives Implemented aseptic technique in wound care
