Full-time RN Clinical Care Coordinator working with medically fragile Medicaid patients ages newborn to 20 years old. The average caseload is 60 recipients. I complete reviews/approvals for PPEC services (Prescribed Pediatric Extended Care) or send to second level physician review for additional consideration. I provide care coordination, complete assessments, develop and update care plans, provide resources, and schedule multidisciplinary team meetings between me, the parent/guardian, the PPEC staff, and insurance case managers/supervisors to review the current health treatment plans and confirm all needs are being met.

Full-time RN Care Coordinator working with the Adolescent Medicine Clinic and the Teen and Pregnant Clinic at Nationwide Children's Hospital. The average caseload was 50 patients. I assisted in care coordination by scheduling appointments, transportation, developing and updating care plans, completing assessments, providing education and resources to assist in meeting the health and social needs of my patients and families. I was also a mediator between patients and their assigned Medicaid Managed Care Plans and worked to resolve issues, concerns, prior authorizations/denials/appeals.

Full-time RN Care Manager focusing on Maternal/Child, High Risk OB and NICU CareSource members. I also cared for the general medicine and mental health populations. I managed a caseload between 70-250 members including low and high risk pregnant, postpartum, newborn, NICU, general medicine and behavioral health members. I assisted in coordinating care between the insurance company, outside providers and agencies by completing assessments and developing care plans to educate and assist members in meeting their needs and improving overall health.

Full-time Staff and Charge RN in the Labor and Delivery unit. I completed patient triage, admissions and discharges. I performed fetal monitoring, cervical exams, lab work, IV placement and care, medication administration, and assisted with labor, vaginal deliveries and C-Sections, including recovery. I completed initial assessments on newborns and provided lactation support. I cared for low/high risk pregnant mothers and their newborns through the antepartum, intrapartum and postpartum periods. I supervised staff, when working as the shift Charge Nurse, and assisted them with their new patient admissions, deliveries, and discharges.

Full-time Staff RN with the Ohio Department of Rehabilitation and Corrections working with incarcerated women and juvenile females. I worked in the prison clinic. I assisted the providers, including the OBGYN residents, with their weekly clinics and completed the daily nurse triage clinics. I administered medications, assisted the diabetics with blood glucose monitoring/insulin administration, performed non-stress tests on pregnant inmates, completed initial health assessments on incoming inmates, scheduled inmates to see outside specialists, completed rounds on inmates in isolation cells, and made decisions regarding medical emergencies within the institution.
