• Practice, promote, and ensure safety and well being of patient
• Feed, bathe, dress patients
• Take patient vital signs
• Serve meals, make beds, keep rooms clean
• Set up medical equipment and assist with medical procedures
• Answer calls for help and observe changes in patient’s condition or behavior

• Provide case management services to individuals with developmental disabilities/mental retardation
• Provide support to individual in their desired life such as addressing health & safety concerns
• Facilitates the development of the ISP (Individual Service Plan) and yearly budget in order to execute ISP
• Conducts face-to-face contact and assessments with the individual and their circle of support to identify abilities, wants, needs, and deficits through written documentation
• Conducts ongoing monitoring and service coordination activities
• Coordinates, attends, and facilitates meetings and conferences on behalf of participant with the circle of support
• Perform person centered advocacy, outreach, public education and resource development

Responsible for coordinating and supervising the services provided by the agency for participants with developmental or intellectual disabilities.

• Attending ISP meetings
• Conducting admissions and in-taking participants to the agency
• Interacting with participants, families, case managers, and DD staff
• Scheduling services for participants
• Consistent search for workers to work with participants during scheduled hours
• Overseeing DSW invoices and authorization of hours
• Conducting on-site visits for participants to ensure the standards of services are being met
• Creating Quarterly Reports to deliver to case managers and guardians
• Ensuring that DSW's/Caregivers are properly conducting their work according to agency standards
• Being on-call 24/7 to attend to any calls for issues/questions families/DSW's may have
