As Regional Director I supported 15 Sunrise communities in New York.

I performed assessments for all potential client to enter the Assisted Living facility.

I coordinated the care of all resident to ensure their needs were met. I Coordinated care with Hospice services, the need for Home Care , communicated with Physicians regarding all aspects of the resident needs and worked with MDs to ensure all medications were ordered and administered.

I motivated employees through effective communication ,

I performed many community site visits for extensive training of staff, hiring and training all of the Resident Care director for each Sunrise community.

I served as a mentor for all coordinators od care for the Sunrise communities sharing knowledge and expertise while fostering an environment of continuous learning and growth.

I performed extensive survey readiness trainings and visits to meet the regulations of NY State Assisted Living

Quality Assurance. Trained and conducted each community to perform quarterly QA.

• Managed complex Skilled nursing cases conducting initial assessments and coordinating skilled nursing care for all aspects of the residents needs
• Maintained timely documentation of patient progress, facilitating effective communication among the care team members.
• Coordinated services with other healthcare professionals such as physical therapists and social workers, optimizing patient outcomes.
