  • The PACT RN was a new position for a new team at the Children's Hospital of Vermont. Duties included working closely with 2 Pediatric Palliative Care physicians to build the program and meet the families, and assist with coordinating care with specialty services. RN also provided education to hospices in Vermont and Northern NY about caring for children on Hospice and supporting those families by doing co-visits with the hospice nurses. RN also provided bereavement services for families after the death of a child.
  • I had the opportunity to work with pediatric patients who had progressive, chronic diseases who presented at the end of life, requiring repeated hospitalizations and goals of care discussions, working with interdisciplinary teams from specialty hospitals in discharge planning and transition to hospice with concurrent care. I also had the opportunity to work with state programs such as DVHA, including their participation in concurrent care for children and families. My experience in the PICU, palliative care, and hospice was instrumental in understanding the treatment plans needed to support these children and parents.

  • Worked with the ACO to improve care and communication in the community. One example of this is a home diuretic program to decrease ED utilization and hospital readmissions. Better care and treatment of this chronic disease process in the home.
  • I have also worked with our Hospice Medical Director in providing education to our area providers about the benefits of hospice for patients in need of that level of care. As the Hospice Manger I also trained and provided education to our nurses.
