  • Assist patients and families in preparing for discharge by coordinating Interdisciplinary Family Training Days, Family Conferences, setting up follow-up appointments, arranging home care, and/or coordinating transfer to another step down facility.
  • Ensure compliance during tasks that coincide with hospital policies, as well as HIPPA, CARF, and Joint Commission standards
  • Member of Traumatic Brain Injury Unit Length of Stay KPI where the committee's focus is to collect and review data as well as create new ways for the TBI Unit to maintain a lower length of stay for our patient population while maintaining patient's rehab goals and overall satisfaction
  • Assisted the Traumatic Brain Injury Unit Length of Stay KPI in developing Interdisciplinary Training Days where every patient has a family member/caregiver come in within the first 10 days of admission for Interdisciplinary Training. This has allowed the patients, families, and caregivers a time to become oriented to the team, as well as, have a better understanding of expectations for discharge early on in the patient's admission.
  • Member of the Hospital Wide Length of Stay Committee where the focus is on standardizing processes across the hospital to assist in lowering the length of stay for out patient population while maintaining patient's rehab goals and overall satisfactions
  • Assisted the Hospital Wide Length of Stay Committee in developing a process used during Team Conferences called 'Barriers to Discharge Focused Reporting'. This process was developed to streamline conversations, ensure actionable items are being addressed, and support an efficient discharge plan for our patient's throughout the hospital.

  • Conduct comprehensive psychosocial assessments on all patients to assess for needs and to identify barriers to treatment compliance; make appropriate referrals to local community organizations, as well as public, private, or nonprofit state and federal agencies

  • Assist with advance care planning and end-of-life preparation and care including: resources for advanced directives, palliative care and hospice agency referrals
  • Serve on an interdisciplinary family-centered care committee to support initiatives to enhance supports for families throughout their family member’s intensive care hospitalization
  • Developed and lead group for family members of patients in the Neurocritical Intensive Care Unit to create a sense of community while providing various resources related to public health programs in the community
  • Works side by side with Police agencies across the state of Maryland, Baltimore City Mayor’s Office of Homeless Services, SBIRT Peer Recovery Coaches, as well as stakeholders in the Baltimore community to ensure Shock Trauma patients have access to resources as well as grants to assist with adjusting to life after trauma
  • Collected and input data for Accountable Health Communities (AHC) Health-Related Social Needs Screening to assist patients with receiving referrals to community programs
  • Complete SAFE-T assessments on patients who have been screened to have possible risks for suicide. Assessment includes identifying risk and protective factors, conducting an inquiry into patient’s current and past thoughts regarding suicide, determining a risk level and interventions pertaining to suicide prevention.
