  • The undersigned hereby makes application for utility service at the above address and agrees to pay for said services as measured by the utility's meter according to its rate schedule. The applicant agrees to permit authorized agents of the utility free access to the premises of the customer for the purpose of inspecting, reading, repairing, or removing property of utility.
