On 12/24/19, I had the first of Two Lumbar injection back surgeries that was work related.

To explain the gap history, December of 2019, I was employed at The Home Depot. I was assisting a customer load his vehicle with one hundred pound bags of concrete when an incident occurred where I was not at fault, resulting in a back injury that required two separate surgeries.
