Work under the supervision of a RN and performs various services for a patient as necessary to meet the patient’s personal needs promote comfort and to enable the patient to remain in the home by supplementing the patient 's/family's/caregiver's abilities for home management. Meeting safety needs of patient and using equipment safely and properly (footstools, side rails, O2, etc.). Provide or assists patients in all aspects of personal care, including baths, back rubs, oral hygiene, shampoos, changing bed linen as part of the Plan of Care.
• Communicates in a timely manner with the RN regarding patient status, interventions completed, data collected and pertinent input to facilitate care planning
• Answers call lights, phones, or intercom system to determine patients' needs
• Serves and collects food trays and feeds patients requiring help
• Provides appropriate assistance to the patient during his/her activities of daily living
• Prepares patients for surgery and/or procedures properly
• Turns and repositions patients using safe patient handling techniques
• Obtains vital patient information as directed (i.e., temperature, blood pressure, pulse and respiration rates, and food/fluid intake/output, and calorie counts)
Support individuals who have developmental disabilities, mental health conditions, medical care needs, and/or physical disabilities in making day-to-day choices that lead to greater independence and community participation. Provide personal and/or medical assistance as needed, teach and role model appropriate behaviors and daily living skills, and provide behavior management assistance.
