  • Developed a Nurse Triage program to help manage acuity of patients transferring around the greater Charlotte area within Atrium Health during the peak of COVID.
  • Acknowledged a need to expand the Patient Logistics Coordinator position, successfully hired and onboarded three more employees to expand covered hours
  • Monitor multiple patients in several different queues for changes in acuity and alert physicians to those changes appropriately
  • Developed and revised based on CMC leadership feedback a shared Excel workbook to track patients being accepted into CMC. Spreadsheet was used daily during the leadership meeting to discuss and appropriately triage patients needs. Patient Logistics RN would present patients to leadership team and answer any questions they may have.
  • Collaborate with Physicians daily to ensure patients are receiving the best care possible and that transfer was still necessary to another facility.
  • Developed a Round-Trip procedure program to help address capacity restraints experienced at all Atrium Health Hospitals. Patients were now able to come for IR, ERCP, and Radiology scans to one of the larger hospitals and return back to referring facility following procedure. Decreased need for admissions to CMC.
  • Served as a educational resource person for questions related to triage protocol or procedure development.
  • Patient Logistics Coordinators/ Triage RN program has been selected as a Bronze Touchstone recipient of 2024 for the work done with CMC Leadership to successfully decrease the rate of admissions into CMC. This was successfully completed by working with referring MD's and Specialty MD's utilizing round-trip procedures, redirecting patients to other Atrium Health facilities with the same specialties, or working to get patient discharged and set up with outpatient follow-up.
