· Develop and maintain professional and cooperative relationships with individuals, families, guardians, providers, DDS Service Coordinators and the internal team at Sunshine Village
· Coordinate, development, and implementation of the DHSP/ISP.
· Maintaining electronic case records for assigned individuals ensuring all information is current, accurate and complete.
· Create a meaningful and engaging daily activity schedule for everyone in conjunction with the program manager.
· Responsible for completing incident and restraint reports within the established guidelines by HCSIS and Mass Health and internal electronic records-eHana.
· Maintain finances of individual accounts per SSV policies and petty cash assigned.
· Work collaboratively with the Program Manager and Program Supervisor.
· Provide direct support to individuals and act as a resource for staff.
· Act as a professional role-model and mentor to directly support employees.
