Description

Working in the Manitowoc County Human Services Clinical Services Division of the Human Services Department, this position facilitates services for adults, children and adolescents who have been diagnosed with a mental illness and/or substance use disorders. This position:

• Performs functional screens and comprehensive bio-psycho social assessments for children’s referrals to determine eligibility and service needs of the child.
• Acts as service facilitator by developing teams of formal and informal supports, collaborating with schools, parent's, foster parents and service providers, developing service plans, make referrals to other agencies and coordinating contracted services.
• Provides ongoing case management and service coordination to families and children enrolled in CCS.
• Develops crisis plans and provides crisis services.
• Facilitates referral, admission, transfer and discharge arrangements with inpatient and residential facilities and providers.
• Generates and maintains clinical records and reports per state standards and agency policy
• Perform timely documentation within 2 business days and maintain required productivity rates.

• Used skills such as active listening and collaborative problem-solving daily to connect with patients and address concerns.
• Evaluated risk to client or others by gathering information and conducting assessments.
• Built safety plans to lower risk of crisis and intervened in specific manners outlined by procedures.
• Conversed with individuals in crisis.
• Provided mental health, AODA, CPS, CFS, YFS and APS after hours coverage.

• Built solid and trusting rapport with children and families, fostering trust and communication to meet case needs.
• Documented data and completed accurate updates to case records.
• Assisted families in navigating the social services system, including accessing resources and connecting with other family support services.
• Maintained detailed case histories, metrics and reports to meet strict standards.
