
Conducts telephone assessments to determine eligibility for home-delivered meals.
Document assessment results, intake interviews, and client status updates in an accurate and timely manner into the database.
Provide support to seniors by assessing safety and living conditions and connecting them to internal and/or external community agencies and services.
Monitor and follow up on client and/or client family concerns regarding missed meals, changes in dietary needs, etc.
Observe and report changes in clients' conditions to appropriate personnel.
