• Provide admission, case management, and follow-up skilled nursing visits for home health patients.
• Administer on-going care and case management for 80+ home bound high risk patients including coordination with physicians and members of the interdisciplinary team.
• Implement and maintain patient’s plan of care with patient, family, and interdisciplinary team.
• Coordinate/oversee/supervise the work of LPNs as part of the patient care team.
• Discharge planning and coordination.
• Prepare appropriate medical documentation in patients EMR.

Care for 1-4 critically ill or prematurely born infants including,

• Hands on care
• Administering medications, treatments, and therapies, Monitoring vital signs and adjusting medical equipment settings,
• Educating and guiding parents on caring for their baby
• Coordinating care plans with other NICU staff.
• Documenting patient condition and all interventions.

Provided hands on patient care in the Maternal- child unit, NICU, and Pediatric Urgent care

Care for 1-4 critically ill or prematurely born infants including,

• Hands on care
• Administering medications, treatments, and therapies, Monitoring vital signs and adjusting medical equipment settings,
• Educating and guiding parents on caring for their baby
• Coordinating care plans with other NICU staff.
• Designated Day shift charge nurse providing leadership and support for staff and coordinate daily flow of unit.

• Care for 6-10 medical -surgical patient with variety of chronic and acute conditions
