•Perform all work in a manner consistent with the National Youth Advocate Program’s mission, vision and values.
•Supporting case managers, clinicians, medical coordinators and other designated staff in completing intakes and documentation, performing assessments, uploading documents, and managing UC Portal database entries, maintaining physical files and ensuring that ORR Manual of Procedures (MAP) and Policy requirements are maintained within the mandatory timeframe, and ensuring children have required supervision.
•Supporting medical coordination team. Attending IME appointments with children, supervising children during medical visits, collecting health records and ensuring IMEs are thoroughly completed, taking children to and from medical appointments, providing child supervision during medical appointments, entering information in UC Portal Health Tab and ensuring proper uploads of medical documentation in files section of UC Portal, ensuring and complying with confidentiality of health records, maintaining UC health files, communicating with external medical providers and pharmacies to ensure that health services are effectively delivered to children.
•Working case assignments under the directive/ guidance of Lead Case managers including interviewing UC, sponsor and family members, completing required assessments, submitting cases for home studies and post release services, completing release requests, and performing additional case management duties under MAP Section 2requirements.
•Assisting in effecting reunifications of children and sponsors. Coordinating with the discharge team, foster parents, and sponsor families about all logistics associated with reunifying the minors with their sponsors, including making travel arrangements (may include booking airfare, hotel, and rental car), coordinating exchanges, ensuring all discharge documentation is completed, provided to, and signed by sponsor.
•Traveling with minors to help reunify with foster or sponsor families. Such travel may include out-of-state travel, airline travel, and overnight stays.
•Working on-call schedules which includes weekends and holidays, with minimal supervision.
