- Works collaboratively with the multidisciplinary team to ensure routine and comprehensive support to transplant patients.

- Monitors patients closely for signs of rejection, infection, and other post transplant complications.

- Educates patients on post transplant self-care, transplant procedures, follow up care, medications, and lifestyle modifications.

-Monitors patients in the immediate post transplant period, assessing vital signs, fluid balance, labs, and wound healing.

-Document patient assessments, interventions, and outcomes accurately and in compliance with regulatory standards.

• Teaches weekly education to new patients and families about the expected evaluation and waiting phases, and long-term follow-up and care
• Manages 100+ patients to ensure timely evaluation, approval, and listing of transplant patients
• Assesses pre- transplant patients and their general health status and collaboratively determines the need for additional intervention according to physician recommendation and current transplant policies
• Facilitates the scheduling of studies or procedures for diagnostic purposes to determine candidacy for transplant
• Completes necessary documentation according to the United Network of Organ Sharing (UNOS) guidelines including listing and status changes
• Participates and leads team conferences, rounds, or meetings in relation to care of the transplant patient or program issues
• Analyzes and reviews medical records to ensure each patient meets criteria for transplant according to company policies and UNOS directed policies
• Experience with hybrid and remote work schedules.

• Assess, plan and document care provided according to facility policy
• Provide critical medications including; sedatives, paralytics, vasopressors, insulin, blood products, and antibiotics
• Communicate with all levels of staff to provide best outcomes to patients
• Proning of patients experiencing ARDS
• Communicate plan of care to family members daily
• Provide comfort care for patients at the end of life.

• Provide care to critical patients in a 20 bed Medical ICU
• Communicate and educate family members on patient’s condition
• Notify physicians of acute changes based on patient presentations and vital signs
• Provide end of life care in a compassionate manner
• Manage and monitor sedatives, vasoactive drips, cardiac drips, and mechanical ventilation both endotracheal and tracheal
• Conduct frequent assessments
• Care and manage arterial lines and central lines.

• Collaborated with other ER professionals (MDs, NPs, Pharmacy, nurses, etc.…) to ensure effective delivery of patient care
