
Advanced Practice Nurse Practitioner with 17 years of Pediatric Nursing experience working with interdisciplinary teams to provide a range of services to manage acute and chronic illnesses. Serve as a liaison and patient advocate between patients’ medical interdisciplinary team, family, providers, facilities, and hospitals. Participate in collaborative process of case management for chronic disease patients including discharge planning and continuing care. Focus on care coordination, screening/assessing/monitoring, plan of care, communication, and reassessment of individual patients’ case by case. Comfortable with assessment, screenings, treatment and follow up of multiple psychological, behavioral, and mental health diagnoses in the pediatric population. Experience with ICD-9 and ICD-10, as well as CPT coding and chart review. Participate in on-going training programs to ensure professional quality performance.
