• Conduct social work assessments and provide support to patients/families throughout various units of the hospital. Provide counseling , connect patients with resources, communicate with care team. Complete mandatory reporting as necessary.

• Conduct social work assessments and provide support to patients/families in the cardiac intensive care, cardiac surgery units, cardiac step-down units, and spinal surgery units. Enter all assessments and notes into Lifechart.

• Assist with running a weekly group for students with anxiety and/or depression.
• Assist with running a weekly group for students with autism to learn and practice social skills.
• Counsel students individually on a weekly basis
• Assist with Individualized Education Plan reviews by conducting social histories with families, conducting classroom observations, administering Vineland rating scales, writing counseling goals, preparing reports for findings, and reporting findings at team meetings.

· Recruit, screen, train, and coordinate volunteers for new Hospice program.

· Prepare and maintain all volunteer files and state required paperwork for volunteer program.

• Manage the activity department for a 72-bed skilled nursing facility.
• Create an activity program that enhances the residents’ quality of life.
• Complete assessments for every resident at move in and quarterly thereafter.
• Coordinate family functions and fundraising efforts.

• Manage a 26-bed memory care neighborhood within an assisted living facility.
• Supervise all staff, and day-to-day operations, to ensure residents receive the highest quality of care.
• Serve as the liaison for family members of residents living within the memory care unit: review care assessments, provide support and education about dementia, resolve concerns.
• Hire and provide training for staff on how to best care for residents with memory impairment.

• Investigate DEA protective service referrals.
• Provide case management services for Neighborhood Health Plan of RI’s Medicaid clients so that they receive the necessary long-term services and support needed to live independently.

• Conduct group activities for the dementia residents of an assisted living facility.
