Gasconade Manor Nursing Home Skilled Nursing Rehab

During my 7.5 years with Gasconade Manor, I
advanced in roles many times, after 1 year I was
promoted to Department Head, and held this title
until my resignation. Gasconade Manor is a skilled
nursing facility, managed by a wonderful team of 10
department heads, with supervision of Administrator.
Below are each of the rolls I carried, with job
description included
Admission Director:
Communicate with hospital case managers at
multiple hospitals to coordinating new patient
admissions to skilled nursing facility.
Reviewing referral from hospital, including
diagnosis of current hospital stay, medication
cost,medical history, etc, with admission team to
determine if SNF can make this patient needs,
cost vs revenue, report decision back to case
manager.
Completed all new patient paperwork/vaccine
policies with new admission/ or DPOA.
Verifying insurance eligibility with patient
insurance company, if out network, obtaining
authorization or -GAP per patient insurance.
Weekly Medicare meeting with all appropriate
staff members in attendance, going over MCR
guidelines, days of inpatient stay and ise patient is
progressing with OT/ PT/ ST/ RT.
Contacting family with progress of inpatient stay
Patient Care Coordinator:
Rehab patients admit to SNF following 3 midnight
hospitalization, verifying all qualifying needs to
meet Medicare guidelines.
Organized discharge planning for home care and
rehabilitation care stays to ease patients'
transition from SNF.
Tracking patient from admission to discharge, to
start the successful discharge with no
readmissions to hospital.
Arranging Home Health, Nursing, Physical
Therapy, Occupational Therapy, Wound Care, fax
in house doctor @ Cap Region with request, fax
referral to preferred HH company and
coordinated the start of in home treatment.
Faxing pharmacy orders for medication needed
for discharge, after in house doctor doctor signs
off on my request.
Ordering DME needed, with all proper
documentation sent to DME company for
insurance to cover orders. Faxing Dr to get
approval and order for appropriate DME.
Creating Monthly Summary of all new
admission/discharges/hospital leaves/ expired
patients, with description of hospital
transfer/diagnosis, and sending over to
Ombudsman Region 6 for review.
Facility Assessment, Discharge/Admission portions
of assessment to be completed and turned into
Administrator for The State of Missouri Annual
Facility Inspection.
Accounts Receivable/ Aging Analyst/ Billing
Creating statements for inpatient/outpatient
each month by deadline, mailing to responsible
party, charge calculator. Billing complete and to
post office by 1st of each month.
Prior to creation, verifying all demographics and
insurance set up is correct, to prevent statement
error.
Aging- contacting any balance
inpatient/outpatient/discharges held with facility,
following balance until paid in full. If unable to
pay in full, starting payment plan,notarize, larger
balances- contacting facility attorney, preparing
the balance summary, description/dates for
collection and follow through until paid in full.
Resident Funds- All residents that wish to hold
cash at facility, cash check and deposit into
resident account, record all deposits/
transactions, monthly and quarterly statement
summary sent to responsible party for signature,
return to facility to be filed into patient account,
per state guidelines.
Maintained accurate records for all banking and
tax purposes.
Deposited all checks/money orders/cash
incoming to facility, and post each payment to
patient accounts.
Radiology billing portal, reviewing inpatient
insurance during date of service, verifying if
invoice is to be sent to facility of patient
insurance, documenting and sending final
invoice to Administrator.
Answer HMO & non traditional Medicare
insurance with counter offer of payment, signing
and return offer accepted to insurance.
Sequence all new patient admissions diagnosis
codes/ adding new codes when needed/
selecting or adding most appropriate dx code
per hospital paperwork/ following PDPM billing
guidelines.
Completing Medicaid Application Process for
long term care benefits with patient/DPOA, follow
through until approval of application is received(3
months)
Entered all FA465's for accurate billing setup.
Medicaid Active patients- notified patient/
responsible party of renewal date 2 months prior
to annual renewal deadline- Complete all
insurance audits/ HMO, PPO and Medicare.
Complete all insurance appeals for
overpayment/ HMO, PPO and Medicare.
Inpatient Satisfaction surveys, collect all surveys
until quarter is over, turning into Administrator to
present to board of directors.
Coordinated with insurance companies for
accurate billing and claim processing, reducing
errors and financial discrepancies.
Assisted patients with completing necessary
paperwork and forms to provide healthcare
facilities with essential information to bill for
services.
Conducted regular audits of documentation
accuracy within facility
Ensured regulatory compliance by staying
up-to-date on industry standards and best
practices related to patient care coordination.
Followed document protocols to safeguard
confidentiality of patient records..
Monitored patients' vital signs.
Conducted thorough intake assessments for new
arrivals, ensuring proper documentation of health
history, diagnosis codes ICD10.
