Setting - Outpatient

Under the supervision of my CI (PT) I performed chart review, chose and implemented appropriate interventions for my patients. Creating treatment plans and having the ability to adapt them on the fly as needed. I have keen listening skills and am able to effectively interpret both verbal and nonverbal signs of pain and exertion.

Knowledge and experience with TheraOffice documentation and billing system.

Setting - Acute Care

Extensive chart review and attention to detail for complex patient management. Keen attention to pt vital signs and nonverbal signs of pain for appropriate patient management. Excellent communication skills with patients, patient families, and inter/intra professional collaboration as needed, both verbal and written.

Under the supervision of my CI (PTA) I chose and implemented appropriate treatment interventions for individuals in the hospital for acute care in trauma unit, and the sidemen cancer center.

Knowledge and experience using the EPIC system.

Setting - SNF

Under the direct supervision of my CI (PTA), I planned and conducted both individual and group treatments for my patients. This included chart review, treatment selection for muscle strengthening, coordination, mobility, sensory awareness, transfer training, balance activities, gait training, range of motion testing and stretching.

I was responsible for monitoring the conditions of my patients though their vital signs, and adapting their treatment according to their response to the interventions I chose. I provided physical therapy assisting services to individuals at the facility for both short and long term care.

Knowledge and experience with NetHealth documentation system.
