- Nephrology Team Lead, providing guidance and oversight.
- Serve as secondary support for Transplant related tasks.
- Act as the primary liaison for the patient-family.
- Responsible for communication and follow-up with primary and specialty medical providers.
- Address the importance of preventive care and the need for appropriate follow-up with health care professionals, rendering care with focus on needed quality measures.
- Provide care coordination, education, and counseling as appropriate.
- Identify and prioritize potential utilization or continuity of care issues.
- Complete Clinical Social Assessments regularly and discuss life planning activities.
- Recognize when care is potentially compromised or delayed, and communicate findings.
- Document and effectively communicate problems, intervention, and plans within current electronic medical record.
- Maintain accurate documentation, including routine updates to the plan of care.
- Educate patients on available services and programs, as appropriate.
- Ensure patients receive all needed services in a timely manner.
- High volume phone support.
- Critically analyze, address and solve complex problems.
- Researching and advocating for appropriate assistance resources for patients
- Examine claims records and forms to determine patient insurance coverage
- Verify insurance benefits and analyze claims for medical necessity
