Provide additional support to clients active in treatment programs by connecting them with additional resources to improve their quality of life.

-Document basic support needs, history with specific skill deficits, levels of functioning.

-Assess and prioritize the needs and strengths of the population served.

-Collect information from medical professionals, educators, social workers, and other sources.

-Document notes with detail of description of services provided.

-Attend care meetings.

-Document direct and indirect contacts.

-Coordinate resources and refer to organizations within community.

Provide therapeutic interventions, referral and assessment services to clients in an individual and group setting.

-Conduct referrals, screenings, and assessments as needed to provide the best care for the client.

-Evaluate medical and social needs of the client.

-Prepare and evaluate evidence based, person centered plans to ensure access to medications, treatment, and other services.

-Provide psycho-education and other supports as desired.

-Participate in treatment team meetings.

- Provide family or community-based prevention services including, but not limited to, child protection, adult protection, juvenile justice, guardianship services, adoption services and services to juvenile offenders

-Assess allegations of child, spouse and/or adult abuse and neglect

-Work as a member of a team with paraprofessional and professional staff to ensure unified service delivery

-Develop service objectives and plans, make appropriate referrals, provide and coordinate needed services

-Recruit and approve adoptive, foster care and family care home applicants

-Assist in the placement of children or juveniles in adoptive, foster care and family care homes or juvenile facilities
