• Key CHW duties include the following but are not limited to:
• Support community-based COVID-19 response including outreach and education (e.g. Flyering, tabling, mask distribution, community wellness activities)
• Build community trust through service-based work and relationship-building
• Help people understand their health condition(s) and develop strategies to improve their health and well-being
• Deliver health information using culturally appropriate terms and concepts
• Link people to health care and social service resources including health insurance, food, housing, quality care and health information
• Provide informal counseling, support, and follow-up, and build individuals and community’s capacity to advocate for needs and services
• Soliciting and capturing community feedback, particularly on community strengths and needs
• Participate in group advocacy and community mobilization activities of the program and serve as a positive example and representative of the organization internally and externally.

ESSENTIAL FUNCTIONS/RESPONSIBILITIES

• Responds to phone inquiries in a timely, efficient, and professional manner by providing resolution within established standards.
• This is a fast-paced call center environment requiring the ability to multi-task and work collaboratively.
• Candidates must be comfortable using technology working in multiple systems at the same time
• Performs research during and after calls as needed to resolve problems and inquiries
• May act as a back-up for inquiries from higher revenue and contractual clients
• Document call notes ensuring key data is accurately captured in system
• Must be flexible regarding both schedule and daily duties.
• Overtime hours may be required based on business needs with advance notice.

• Nurse Liaisons are responsible for advocating and providing assistance for patients in a health care facility. Their duties include assessing medical records and patient eligibility, connecting a physician for a patient, assisting in the admissions process for patients and family, and addressing any queries or concerns. Besides that, they must maintain consistent communication with physicians as well as external and internal case managers. Nurse liaisons may also assist in the patient’s precertification process relating to commercial insurance or government health plans.
• Supports patients by providing housekeeping and laundry services, shopping for food and other household requirements, preparing and serving meals and snacks, and running errands. Assists patients by providing personal services, such as, bathing, dressing, and grooming.
