- Reviewed and tracked discharge plans, care plans, and progress notes with an interdisciplinary team every morning.
- Fostered rapport with clients and their families, and referred home care and medical resources to ensure continuity of care.
- Reported the well-being of clients to health care professionals through written and oral communications.
