• Collaborating and assessing needs of the patient and patients family, when appropriate, and arranges, coordinates, monitors, evaluates, and advocates for patients.

• Direct client contacts, observing families, ensuring the safety of children, investigative assessments, and assisting them in contacting providers for positive reinforcement.

• Provided Case Management services including: Hospice coordinator, Discharge Planning, Educating Staff/Residents on Resident’s Rights, Care Planning, and Interviews for MDS Assessments, Admissions, Grievances, Resident Counseling, supervise podiatry/eye care services, and Completing FL2 with NC Tracks.
