Dedicated and passionate social worker seeking a position with the opportunity to enhance effective client engagement skills and clinical knowledge. Substantial experience in providing needed support, resources and ongoing advocacy to families, adults, and children. Fluent in managing multifaceted caseloads, coordinating treatment plans, and facilitating appropriate referrals. Ability to show leadership and effective problem-solving skills in time sensitive and crisis situations. Knowledge regarding available services and requirements within various mental health levels of care.
• Demonstrate knowledge, skills and abilities required by the population served. Competence shall be demonstrated by exhibiting core skills including: technical knowledge; cultural awareness; analytical skills;decision-making skills; interpersonal skills; communication skills; supervision skills; and clinical skills.
• Coordinate and monitor services on behalf of the consumer. These services may be provided in any location and may be provided to an individual or a group of individuals. This includes telephone time with the consumer and collateral contact with persons who assist the consumer in
meeting his/her rehabilitation goals, linkage of consumer/family to services/resources, development and maintenance of the consumer's person centered plan, coordination and facilitation of team meetings, completion of a minimum of two (2) contacts per month with one (1)contact occurring face-to-face with the consumer.
• Facilitate (or work with case manager to facilitate) consumer's access to services through the authorization process and by completing all needed paperwork associated with this process.
• Completion o f initial and ongoing NC-TOPPS assessments, yearly I R S matrix, and all other paperwork required by the ACS, the Division, and the State.
• Ensure that services are provided in a manner which meets all applicable regulatory licensure and accrediting regulations.
• Provide or supervise direct interventions including various skill building activities for daily living,community living, socialization, adaptation, and symptom management; wellness education and substance abuse education; and behavior and anger management techniques.
• Participate in routine staff meetings, attend all appropriate in-services, training workshops, or continuing education programs.
• Ensure that consumers' medical record charts are current and contain all necessary information.
•Work as requested to provide mental health outreach, education, and prevention in the community.
•Complete or participate in appropriate intakes, assessments, and orientation for new consumers.
•Maintain strict confidentiality according to all Federal and State guidelines and requirements.
•Maintain positive personal and professional conduct at all times.
•Uphold the Professional Code of Ethics.
•Maintain a cooperative relationship with employees, medical staff, and others. Adhere to and support ACS policies, programs, and activities.
•Demonstrates tact, courtesy, and a positive approach to communication and interaction with other employees, consumers, community partners, etc.
•Continue personal development to ensure current knowledge in the field of service.
•Maintain a safe, clean, and drug free environment.
•Display a willingness to work beyond what is required periodically and travel for continuing education hours.
•Maintain service documentation that meets ACS standards, Medicaid standards, and/or any other auditing agency's standards, such as the LME. Submit all documentation and other components of consumers' medical records within agency timelines. All service documentation must include:
1) the purpose of contact; 2) describes the provider's interventions; 3) effectiveness of the intervention; 4) the time spent performing the intervention; and 5) signature (degree/credentials or position) of person providing service.
•Submit accurate monthly timesheets detailing services provided in the manner prescribed by current agency policy.
•Be available in the on-call rotation for 24/7- 365 days per year first responder coverage.
•Maintain the flexibility to respond to consumer needs and crisis situations beyond the agencies normal working hours.
•Perform any other job-related duties as specified by supervisor.
•In addition to these activities the clinician must be able to adapt to the changing nature of the abovementioned requirements. He/she must be able to adapt to changing paperwork requirements, changing forms, and frequently changing expectations.
