As a Post-Release Service Case Manager and Bilingual Case Specialist, I am dedicated to staying in compliance with both internal and external standards, as well as the requirements set by accrediting agencies. I have conducted over 200 initial intake sessions with new subjects admitted to the program, where I familiarize them with program requirements, examine their criminal files, and create preliminary treatment plans. The role involves coordinating high-quality care for unaccompanied minors and diverse patient groups, addressing specific client needs based on individualized treatment plans. Additionally, I am responsible for collecting data on treatment goals and ensuring that all aspects of care meet established norms and processes.
