Examples of related forms include, but are not limited to Rights & Obligations, updated Voter Registration, Custody Paperwork, etc. Monitoring will be based on chart reviews, the Out of State Transfer Report, QA monitoring/reports by staff.
Provides low risk nutrition education contact and completes data entry of 8011 contact code in FL-WiSE for WIC clients. FL-WiSE service code data documents monthly number of clients provided 2nd low risk nutrition education contact.

Monitoring will be through a minimum of 4 observations, monthly high risk reports, chart reviews, processing standards, QA staff monitoring/reports, and observations.
