Duties include case management of members in payor programs Cigna and Aetna using predictive modeling to identify patients most at risk of hospital admission or readmission. During the review of charts various quality metrics are used to recognize opportunities to assist the patients and the clinics. I have also participated in many payor specific projects such as Cigna Grand Rounds and Cigna catheter reduction. Other duties include education opportunities with patients as well as clinic nurses and managers.
