*Assists in overall development, coordination and delivery of Sexual Health & Pre-Exposure Prophylaxis (PrEP) Clinic services-including STI testing, counseling, prevention education, treatment/treatment adherence, partner services, linkage to care and reporting

*Assists with the preparation and monitoring of grant application, budget and timely reporting of deliverables; Assists in providing oversight of Sexual Health/PrEP Clinics; Ensures clinic schedule is prepared and verified; Ensures adequate staffing and support; Orders and prepares needed supplies; Coordinates care and implements follow-up; Secures records

*Assists in creating collaborations and partnerships with health care providers, clinic managers, and other medical professionals to assess and assist with service delivery; Provides consultation and technical assistance as needed

*Assists in providing divisional support for Sexual Health/PrEP education, referrals, testing, partner services, linkages to care and reporting

*Assists with community education with key points of service entry options

*Follows-up on all surveillance cases in the 19-county region assigned by IL Dept of Public Health-those who are newly diagnosed with HIV, a STI co-infection or an unsuppressed viral load to offer linkage to care, partner services, education and supportive services

*Documents all activities in the PROVIDE database system

*Responds to case manager referrals for Partner Counseling and Referral Services

*Collaborates and partners with health care providers to assist with service delivery by providing presentations, tabling at events and arranging for shadowing opportunities.

*Provides Community outreach education for referral and linkages to care

*Assists Medical Case Managers with clients that are at risk or close to be closed

*Assists in follow-up with client goals/barriers.

*Tracks linkages, viral suppression and other referrals needed by clients

*Tracks all client contacts through PROVIDE database system

*Implements assigned responsibilities and directs other staff as part of the agency's emergency response for both exercise actual responses.

*Incorporates ethical standards of practice into all interactions, including use of data and information

*Adheres to HIPAA privacy regulations and OSHA safety standards pertaining to exposure to toxic substances or other hazards.
