Clinical Case Manager

My position is to proivde community and clinical based interventions to support and assist clients with mental health diagnosis. My role is specific to an outpaitent level of care for clients seeking Case Management Supports. 

  • I outreach clients at their homes when needed as well as provide community based services including transportation to and from appointments with other providers if needed. 
  • Providing skills training, connection to community resources, and to peer supports. 
  • It is also my duty to advocate for my clients to the best of my ability to get them the support they need. 

Lead QMHA

My role as a lead QMHA has been to assist in the opening of a new Residential Treatment Home. 

  • I have helped create a group curriculum to teach ADL's to clients by putting together a year long treatment schedule.
  • Partaking in the interview process for open DCS positions as well as meeting with community partners.
  • Conducted assessment interviews to evaluate clients’ mental health issues and needs

Pre-Treatment Clinical Case Manager

Working at Sequoia I provided case management and skills training services as well as being apart of a pre-treatment pilot program.

  • This has been for clients with an SPMI diagnosis and for clients not yet enrolled in services through a pre-treatment program.
  • I work one-on-one with individuals to assist them in setting and achieving goals they would normally be unable to achieve on their own due to the barriers created by their mental health.
  • Conducted home visits and community outings to promote socialization and independence
  • Support the intake department with phone screenings, scheduling initial appointments. insurance checks, and care coordination.
