The Case Aide is directly assigned to the home study and post-release program and is responsible for assisting in the day-to-day tasks to ensure that services are provided to youth in a timely manner.

  • Must ensure that youth are properly enrolled in the program's internal intake system (ETO, UAC portal).
  • Audit ETO and UAC portals to ensure compliance and accuracy.
  • Assist with accepting referrals, managing active case logs, and contacting sponsors and families as directed by Regional Lead or designee.
  • Maintain data on all clients enrolled in programs and all dismissed clients.
  • Contact youth and sponsors as directed by Regional Lead or designee.
  • Document all actions taken and contacts with youth, sponsor, and stakeholders informed of progress notes (touchpoints) as required by SWK, State, and Federal contracts.
  • Assist with coordination of travel arrangements for program staff for purpose of completing home visits (may include booking airfare, hotel, and rental car).
  • Assist program staff with preparing home visit documentation including locating case-specific resources/services prior to scheduled home visit.
  • Assist with completing sponsor surveys as directed by Regional Lead or designee.
  • Assist with filing, closing, and overall daily maintenance of records found in either physical case files or on ETO or UAC Portal software.
  • Assist with making referrals to TVAP and OTIP.
  • Assist with managing and updating resource spreadsheet.

The YCW Escort is responsible for the line of sight supervision of unaccompanied minor(s) and families in accordance with established transportation by land and air policies and procedures. All services shall be provided in accordance with established standards, principles and ethics of the profession.

  • Responsible for the safe and secure transportation of unaccompanied minor(s) and families in a humane manner in accordance with established transportation by land and air policies and procedures.
  • Ensures that an orientation is provided to all children and families in a language they understand.
  • Reviews the documentation to ensure it contains all information needed for travel.
  • Completes, verifies, and provides documentation to all parties involved.
  • Prepares to work with uncooperative individuals in both controlled and uncontrolled situations.
  • Prepares meals and snacks for the children and/or family units for the duration of the transport.
  • Logs transportation details, such as medication, meals offered, eaten, and refused.
  • Provides clean and appropriate clothing based upon weather conditions at destination location.
  • Reports into the Command Center(s) at established check points and advises on any changes, disruptions or concerns throughout the transportation route.
  • While travelling, the YCWE, may incur out-of-pocket expenses. These expenses will be reimbursed by the company as long as proper documentation is presented (receipt and expense report).
    Travel Chaperone will be required to provide documentation of all duties performed while on shift.

Youth Care Worker

As Youth Care Worker, I am responsible for providing direct care supervision for youth residing in Southwest Key Programs (SWK) care fostering environment that facilitates learning and behavioral management in a supportive manner. Youth Care Workers will maintain a flexible, organized, and efficient work schedule.

  • Conduct intake services to include but not limited to completing the necessary paperwork, and providing youth with an overview of program services for initial placements of youth into the shelter.

  • Assist with the planning and coordination of special educational events (to include but not limited to: spelling bee, science fair), acculturation activities (Cesar Chavez Day, Thanksgiving, Chinese New Year, Holi-Festival of Colors, 16th of September), educational (to include but not limited to: visits to museums) and recreational outings (to include but not limited to: movies, park, amusement parks), assist in the coordination of student council activities and other meaningful experiences for SWK youth which promotes volunteerism and learning.
  • Provide vocational courses and curriculum in a subject area approved by the program director or designee that introduces youth to career choices.
  • Assist with the coordination of Physical Education instruction and other large muscle activities in coordination with the Program Director or designee as required by state and federal requirements.
  • Must assist in evacuating of youth as needed due to inclement weather conditions, natural disasters, or other unforeseen occurrences.
  • Observe, report, and document identified behavior patterns and required information that merit documentation such as Elevated supervision forms, close observations, food logs, sleep logs, and bed check logs.
  • May be asked to provide personal care services to infants and toddlers in a childcare setting. Perform a variety of tasks including but not limited to: dressing, feeding, bathing, and overseeing toddler's play. (Only for region programs with children from 0-17 years of age and certified personal care staff only)
