• I am a licensed doctor who has evaluated the emotional profile of Marisol Albino and determined Marisol qualifies for an Emotional Support Animal (ESA). Marisol is experiencing high levels of stress. Marisol’s high stress levels have imposed limitations in quality of life and daily functioning. Marisol’s distress is imposing a negative influence over lifestyle. In light of Marisol’s disruptive stress level, I believe the stress is such that Marisol Albino will benefit from having an Emotional Support Animal. The animal will help provide a safe and more consistent emotional support system. Having Emotional Support Animal will provide Marisol a constructive soothing stress management solution in order to help keep calmness and to help prevent any possibilities of escalating stress that could cause further complications to overall well-being.
• The emotional distress that Marisol experiences is such, that I have recommended their pet be recognized as an “Emotional Support Animal” and be allowed to live with Marisol under the United States Fair Housing Amendment Act of 1988. To help Marisol cope with stress, enhance ability to function and live happier to fully enjoy the dwelling you own or manage, Marisol should be able to keep a Chihuahua (Dog) named Linda that weighs 2 lbs. as their Emotional Support Animal.
• It is also recommended that Marisol Albino be allowed to have their pet with them at restaurants, stores, hotels, trains, buses and ride share services.
• I am a licensed doctor who has evaluated the emotional profile of Marisol Albino and has determined that Marisol qualifies to have an Emotional Support Animal (ESA) for Marisol’s psychological disability recognized by the DSM-V. Marisol’s disability substantially limits at least one major life activity.
• It is my professional opinion that the presence of an Emotional Support Animal is necessary to help treat and alleviate Marisol’s disorder. I have recommended Marisol Albino keep their Chihuahua (Dog) named Linda that weighs 2 lbs. to serve as Marisol’s emotional support animal as the primary treatment to address Marisol’s disability.
• Please allow Marisol Albino to be accompanied by Marisol’s Emotional Support Animal in the cabin of the aircraft, in accordance with the Air Carrier Access Act (49 U.S.C. 41705 and 14 C.F.R. 382).
