Manage a patient panel consisting of adults and seniors with a broad array of needs and backgrounds at a busy, fast paced residency continuity clinic (see my own patients as well as urgents every 5 weeks)
Conduct a mix of acute, chronic and well visits including medicare wellness visits
Diagnose, develop and implement evidence-based, patient centered treatment plans for a wide range of acute and chronic illnesses
Coordinate and manage patient care, including referrals to specialists and follow-up care
Provide health coaching and education to patients on disease prevention, lifestyle modifications, and self-care management
Be available by phone for emergency calls
