• Completely and accurately document all Medical Information Requests, Adverse Events and Product Quality Complaints in compliance with Ashfield, Client and Federal guidelines and regulations

Works with the Health and Wellness team as part of the Addiction Medicine Team and in collaboration with the behavioral health treatment team in the provision of medication education, support, and adherence, the monitoring and treatment of participants utilizing Medication Assisted Therapy (MAT) services for those with Opiate Use Disorder (OUD) and other Substance Use Disorders, and coordinates care for the participant with the residential behavioral health team, the addiction medicine team, primary care, and other internal and external healthcare resources for the participant.

● Manages medication storage, administration, and adherence for participants.

● Monitors patient medication use and helps to mitigate risk to patient by flagging high dose medications and potential medication interactions to the Director of Addiction Medicine and/or designee.

● Also support Medication Assisted Treatment through counseling, education, and administration of injectable medications under the supervision of the addiction provider and standing orders.

● Provides appropriate participant care education and counseling.

● Manages prescription refills under the guidance of the addiction provider and standing orders.

● Provides complex care and care coordination for all participants accessing healthcare services internal and external to HealthRIGHT 360.

● Works closely with staff collecting and reporting contractual data as necessary.

● Functions as a nurse within the scope of California BRN licensure and credentialing and privileging of the agency.

Performed admissions, readmissions, discharges, recertification using the OASIS documentation. Coordinated with other skilled clinicians involved in the case (i.e. PT, OT, MSW, ST, HHA) in order to provide what services are appropriate for the patient at home. Provided nursing care in patient’s home ranging from utilizing assessment skills to determine necessary nursing care, obtaining medical history and current medication regime to development and implementation of care plans. Provided needed nursing care, including administering IV antibiotic therapy and medications, IV hydration, performing PICC line site care and wound care (including wound vacs), lab draws via venipuncture or from PICC line, foley catheter insertion and supra-pubic catheter insertion. Also provided necessary education and training for patients including management of pain and chronic diagnoses including heart disease, diabetes and COPD.

• L/D Nurses provide direct and individualized nursing care to patients based on the application of scientific nursing principles. In addition to general nursing care, responsibilities of L/D Nurses include (but are not limited to):
• Sits with and monitors patient throughout (before/during/after) the labor Monitors contractions and helps patient with breathing techniques Performs basic IV and central line skills
• Performs phlebotomy skills Knowledge of hemodynamics
• Administers pain medication as ordered; administers other meds to address specific medical conditions, such as hypertension and diabetes
• Takes and records patient’s vital signs, ascertains if problematic
• Reads fetal monitor & ascertains if there is a problem with baby’s heart rate Identifies & manages life-sustaining physiological functions in unstable patients Checks patient’s cervix periodically to monitor progression/lack of dilation
• Ensures patient’s bed is kept dry & clean; cleans up bodily fluids expelled before & after the birthing process
• Contacts anesthesia when/if patient is ready for epidural; contacts delivering physician at appropriate time, ascertains proper time

• The Kempsville Center provides psychiatrically and medically managed acute and residential treatment for children and adolescents with significant impairments in their social and emotional development.
• The RN provides resident assessments, nursing care and evaluation of nursing care.
• The RN ensures consistent use of the nursing process in providing quality resident care and acts as a resource to resident care staff.
• The RN serves as a role model for residents to observe and learn socially effective values, attitudes and behaviors.
• The RN implements emergency use of seclusion and restraints (i.e. assessing out of control residents for appropriate use of seclusion and restraint, assess resident for needs of PRN medication and notifies on-call MD, etc.).
• The RN assess residents and documents pertinent resident information.
• The RN plans resident treatment (i.e. assisting in the admission, transfer and discharge of residents.).
• The RN intervenes psychotherapeutically with residents (i.e. demonstrating therapeutic communication skills, demonstrating sound clinical judgement and communication skills for managing out of control residents.).
• The RN intervenes with respect to resident's psychopharmacological and physiological needs (i.e. transcribing physician orders, following medication administration policies/procedures, administering medications as ordered; instructing residents in use of medication, monitoring laboratory results and medical consults, maintaining resident's physiological status by administering necessary treatments or interventions and documents the same, performing EKGs as ordered, organizing/assisting with both pediatric and psychiatric rounds, performing initial and annual resident's health screening for tuberculosis and appropriate immunizations.).
