• Deliver family planning services, including contraceptive counseling, prescribing, and ongoing management.
• Perform physical assessments through collaboration with the physician and manage aspects of care appropriate to the Nurse Practitioner’s training.
• Provide reproductive health care for both female and male clients, including:

o Routine screenings such as Pap smears, prostate

exams/screenings, semen analysis, STD and HIV

testing.

o Evaluation and treatment of contraceptive and

sexual health needs.

• Perform DOT exams, sports physicals, school physicals, and Head Start/DCFS physicals in accordance with state and federal guidelines.
• Conduct patient assessments, order and interpret diagnostic/laboratory tests, develop and implement patient management plans, and provide follow-up care.
• Interprets common laboratory and/or diagnostic screening at least to the point of recognizing deviations from the normal.
• Provide education and counseling to patients regarding treatment plans, reproductive health options, anticipatory guidance, and health maintenance.
• Facilitates referrals to local physicians, hospitals, and community or social service agencies, including those managed by Southern Seven Health Department.
• Evaluates clients, documents plan of care, and prescribes contraceptives and treatment for STD’s following program protocol.
• Maintain complete, accurate, and timely health information, progress notes, and patient records. Participate in quality assurance activities, including chart audits, policy reviews, and protocol updates.
• Collaborate with the Medical Director for clinical oversight. Participates in a periodic review of patient health records with the Medical Director.
• Complete required reports and submit them in a timely manner.

• Assists in the determination of eligibility for a wide range of public health and overall agency services.
• Performs professional nursing functions, including the use of interviewing and public health nursing assessment skills, the identification of health, social, and other related needs of the client and family, and determines the urgency of the need for services and initiates appropriate action.
• Assures documentation of service delivery in accordance with professional standards and Southern Seven policies and procedures to ensure completeness and accuracy of client records and associated data bases; uses only agency approved abbreviations.
• Maintains confidentiality of all client records.
• Performs home visits according to agency and Southern Seven policy and procedures.
• Coordinates client activities and referrals within the agency across programs as well as with school and other community agencies as appropriate; maintains knowledge of local and state program policies and procedures.
• Participates in appropriate staff and agency meetings with supervisor’s approval to plan for services and to assure the coordination with community agencies.
• Participates in communicable and chronic disease investigation and surveillance.
• Performs delegated medical functions as outlined in Southern Seven policies and procedures and standing orders (vital signs, heights/weights, TB skin tests, pregnancy tests, blood lead tests, hemoglobin and cholesterol screenings, STD testing, etc.).
• Prepares and administers vaccines for an immunization in accordance with Southern Seven policies and procedures and standing orders.
• Performs lab procedures, including venipuncture and prepares specimens as necessary; conducts follow up on abnormal lab results.
• Complies with all standard operating procedures of Southern Seven regarding employee health and safety, including but not limited to OSHA regulations, CLIA, EPA regulations regarding waste disposal, use of appropriate personal protective equipment, and adheres to Exposure Control Plan.
• Assures that medical supplies and equipment are maintained in an appropriate manner and reports any problems, malfunctions, or supply issues to the Director of Nursing in a timely manner.
• Works at other clinic sites as assigned based on agency needs.
• Assists other nursing or clerical staff as clinic needs warrant.
• Provides direction to paraprofessional staff in delivery of client services.
