  • Responsibilities:
  • · Effective communication as the representative of CBCIH with assigned Community Based Care (CBC) Lead Agencies, foster parents/caregivers, adoptive parents, guardian ad litem, and young adults
  • · Provide technical assistance and training with in person meetings, through Zoom, Teams, phone calls, emails, and texts to the assigned CBC Behavioral Health and Nurse Care Coordinators regarding the Child Welfare Specialty Plan
  • · Compliance monitoring for assigned CBCs with Excel tools to ensure requirements of the contract between CBCHI and Sunshine Health which includes
  • o health care management functions including enrollment, care coordination, utilization management,

  • · Review Key Performance Indicators with assigned CBCs that include target goals for enrollment, Health Risk Assessments, Caseload Documentation, Qualitative Documentation Reviews, and Healthcare Effectiveness Data Information Set (HEDIS). Develop improvement plans with the assigned CBC Care Coordinators and staff to ensure members receive the services they need to achieve positive outcomes.

  • The FCCBHN's mission was to ensure the effective provision of services to youth in the Child Welfare system. The FCCBHN worked to integrate Medicaid managed care into the State of Florida's Child Welfare system.
  • Responsibilities:

  • · Served as the liaison between the CWPMHP and assigned CBCs and their staff, caregivers, members, and provider groups

The Therapeutic Group Home provides residents with 24-hour supervision and instruction while preparing for a less restrictive setting or independent living.
