• Facilitated client access to community resources, including locating housing, food, clothing, school programs, vocational opportunities, or recreational activities, and relevant mental health services; assisted the client in developing natural resources and making contact with social support networks.
• Coordinated and monitored services, including comprehensive tracking of client activities in relation to the care plan.
• Assisted clients in developing goals and assisted in developing treatment plans, which are assessed regularly, conducted medication and mental status assessments, and determined the required level and frequency of service.
• Documented all client encounters and contracts made on behalf of clients, completed and submitted billing documentation as appropriate, and maintained comprehensive client files.
• Identified and provided emergency crisis services as necessary; made immediate clinical assessments and responded according to accepted crisis intervention methods and techniques; coordinated other services as appropriate.

• Attendance at county-wide meetings regarding HUD housing, the development of central intake for supported housing, to obtain project funding, and the disbursement of grant money.
