• Provide information about participant responsibilities, rights and concepts of self-direction

•Assist with developing and implementing the individual budget portion (spending plan) of the Personal Support Plan including assisting the participant to identify risks and develop an individual back-up plan

• Provide clarification and explanation about program requirements and documentation/record keeping for individual budgets

•Coordinate activity with FMSA entity

•Assure participant is receiving individual budget services

• Maintains close liaison with appropriate referral sources, makes timely contact with potential clients; determines eligibility and willingness to participate and advises clients of results.
• Follows appropriate assessment procedures to identify health, functional, cognitive environmental and support needs of client, develops care plan and services contract and enrolls clients in program.
• Maintains continuing contact with client, his/her family and service provider to ensure care plan is followed, client remains eligible and needs are met.
• Maintains required files and records correctly and submits accurate and timely reports.

• Interviewed job applicants to obtain information on work history, education or job skills.
• Assited with scheduling interviews with agencies hiring based on skill set of individual; on site job coaching provided as needed/allowed
• Monitored client participation in assigned employment programs, ensuring compliance with all applicable regulations and standards.
