Provided social service work to the ID community of Washington County. involved with providing support, guidance and oversight monitoring to individuals who have Intellectual Disabilities (ID). Serves in the role of a Supports Coordinator (SC) responsible for locating, coordinating, and monitoring the supports and services for each individual assigned to their caseload. Work is performed in accordance with established mandates and bulletins of the Department of Human Services (DHSW), Office of Developmental Programs (ODP), and the Centers for Medicare & Medicaid Services (CMS), as well as the policies and procedures of the BHDS office.
Assisted individuals in crisis to obtain services, coordination and authorizations for care, and billing. Primarily worked with Mental Health and Intellectual Disabilities individuals.
