Provided assessment and referral services to other community support groups as needed as well as coordinating treatment admissions. Facilitate relationship building group for youth, as well as other evidence based curriculum on a weekly basis. Complete Biological/Psychological/Social Screens to identify referral for Protective Custody youth that enter into the Emergency Shelter program. Responsible for providing clinical support to the YRCII. Counseling individuals or groups regarding psychological or emotional problems such as stress, substance abuse, residential adjustment, or family situations (stress, anxiety, depression, trauma, and residential life.) Perform mental health screenings and assessments. Develop and implement counseling for youth interactions.

• Provide intake assessments and initial treatment planning
• Establish therapist-client relationships and provide therapy services at 8 different schools in the community
• Conducted psychotherapy (individual, family and group) and on-going treatment planning
• Complete all client documentation within appropriate time
• Make referrals outpatient office to include medication management, case management, attendant care, and other services as needed
• Work collaboratively with school administrators and social work staff
• Provide daytime crisis services when needed.

• Support the teacher working with individual or groups of students
• Assigned to meet the needs of specific students and/or program needs within the school
• Work alongside and under the guidance of my supervisor, who is ultimately responsible for the welfare and safety of all students in the school and serve as a support
• The role is to link academic assistance, communication and/or understanding between the school, classroom teacher, families and community by assisting at the IEPs, school gathering or events.

• Help members, who are children on the Serious Emotional Disturbance (SED) Waiver, to get the services and the care that they need
• Educate members more about their conditions
• Work with members and their providers to make a care plan and prevent gaps of care
• Complete assessment with members about their health to assess their needs and find resources.

• Work under supervision of other therapist providing crisis intervention services and pre-admission assessments to meet the needs of members of the community who are in crisis
• Perform clinical assessments for individuals who present as a danger to self or others at Crisis or other community locations
• In addition, completion of intake, follow-up, and outreach appointments, as well as answering crisis telephone lines as needed
• Pre-admission assessments entail evaluating for appropriate level of care
• Support suicide prevention efforts in the community and upholding rules and regulations.

• Provide targeted case management and community psychiatric support treatment for consumers
• Conduct assessments and create collaborative development of consumer goals as outlined by Evidence Based Practices
• In addition, responsible for the coordination of services and assistance needed to meet identified goals each consumer, with the overall objective of rehabilitating the consumer in the community at their highest level of functioning.

• Supervise the recreational, classroom, and daily activities of all youth offenders, while maintaining order and security
• Conducted multiple checks hourly while residents are in their rooms to ensure they stay safe and remain in the facility
• Observe how residents interact with each other, staff, and volunteers
• Preform body searches every time resident returns to the facility or interacts with visitors
• Conduct random inspections of youth’s rooms to locate contraband and ensure areas are clean and safe.
