Provide recovery management services to individuals with severe and persistent mental health diagnosis; along with individuals who were diagnosed with chronic conditions who are enrolled in the Specialized Recovery Services Program.

Perform program assessments and services plans for individuals on the program. Service plans include services provided through Specialized Recovery Services or coordination of other state plan services that are Medicaid and non Medicaid Community based services.

Maintained regular contact program participants either by telephone or in person visits to ensure participants needs are being met.

Conduct comprehensive assessment assisting Veterans and/or their representative to develop a plan of care utilizing formal and informal supports to Veteran's health and safety needs at home.

Educate Veterans, families and community agencies regarding options within the Veteran's Directed Program and other communities resources to access services at home.

Advocate for service delivery through collaborating and negotiating with services at home.

Maintain regular contact, either by telephone or home visit, with the Veteran to determine if services are appropriate, quality of service and meeting the Veteran's need.

Conduct comprehensive reassessment of Veteran yearly- quarterly home visits to evaluate care provided.

Maintain documentation in an accurate, timely and complete record for each Veteran.
