Triage: Vital signs, chief complain, note goals and barriers, record and update medication and allergies. If asthmatic or been seen for ADHD or ER follow up. Have guardian fill out appropriate forms and obtain Medical record from Epic (lee Health) for ER visit or Specialist. If out of state have patient sign a medical release form to obtain medical records. If needed fill out Asthma forms, physical exam form or sports physical exam and provide immunization summary with certificate. Ask patients if translation is needed. Offer all and none mandated immunization.
Laboratory test and phlebotomy finger prick or venipuncture procedures: RSV, Flu, Strep, Corona Virus, Urine dip, glucose, hemoglobin, catheter procedures, pregnancy, STD and drug testing. And any other blood test ordered by provider.
Administrate medications: Oral, inhalation, antibiotic, and contraceptive injections. perform PDD skin test
Administrate Immunizations to children from 0 to 18yrs old.
Assist low income families on completing application for child care. Once approve register the family on Early Learning Coalition and on the program Promis. Assign child to facility or home daycare more convenient closer family or were theres a openings available accommodate family if there is more then one child needing childcare.
Develop a relation with families by facilitating parent meetings once a month with fun ice breakers to have families engage. Set up home visits in a comfortable environment which could include at their home, office or in a public area. Help family create a short or long term goal whether it was individually or collectively.
Attend community agencies meeting to keep family informed with the resources that the community has to offer.
Provide transportation and translation at medical appointments and occupying the parents to IEP meeting who have special needs.
Sign up families for free thanksgiving meal baskets, Christmas gifts and diapers for child from newborn to 2yrs old.
