The Diagnostic Service Coordinator (DSC) is primarily responsible for working with the youth that are placed in the diagnostic program. Working with children to obtain referrals, conduct child interviews, gather collateral information, completion of assessments and observations and work with the treatment team and psychologist. Assumes the leadership role in assessing the needs of the child placed into the diagnostic program. Demonstrates the ability to identify the strengths, challenges, abilities, and potential problem areas of the child in their home and in care. Works closely with the Family Coordinator to facilitate the recommendations for the child and family.
Duties and Responsibilities:
● Part of the admissions team committee.
● Schedule and conduct child Interviews with corresponding assessments.
● Conducts Assessment of the child and their knowledge about the diagnostic process.
● Process and facilitation of the child interview and assessment process.
● Confidential maintenance of the child’s case record, including case notes, documenting on-going meetings, communication with MDT members, school personnel including dates, issues discussed, and outcomes.
● Assisting the psychologist with observations and assessments in order to comprise realist discharge planning.
● Schedule and Facilitate the MDT and recommendations.
● Coordination with Family Coordinator to assist in linkage and referral for community base child and family services.
● Other duties, as deemed necessary by designee.
The Treatment Specialist (TS) provides supportive counseling and treatment planning to youth ages 6 to 14 years of age. The Treatment Specialist completes assessments such as Intake Assessment, CANS Assessment, Human Trafficking Screener, CRAFFT Assessment to evaluate the youth for needed services. The Treatment Specialist works with the treatment teams in meetings to provide services to youth for their identified problems/issues.
Duties and Responsibilities:
● Compilation of assessments including intake and treatment planning.
● Assists in the enhancement of counseling and coping skills.
● Participates in consultation meetings and provide feedback on progress to the Clinical Team.
● Ensures that daily behavioral health services are being met.
The Forensic Case Manager provides social services assistance/case management to patients and their families. Works collaboratively with the treatment team in aspects of patient care; links patients with needed aftercare resources/referrals; discharge planning; completes, assists with, and/or schedules needed assessments; makes collateral contacts; examines case situations and presents information to medical staff and other members of the treatment team regarding patient’s family and social background pertinent to diagnosis and treatment. Works in conjunction with William R. Sharpe Hospital and the courts to get patients to less restrictive environments.

The Admissions Clerk duties include phone triage, addressing requests for services and possible crisis intervention, answers calls with regard to hospital services, bed availability, general inquiries; offers resources when applicable, verifies insurance, utilizes our electronic medical records system, completes intakes and paperwork relevant to intakes. Reports to the Director of Admissions and Referrals.
Duties and Responsibilities:
● Provides clinical screening/assessment for potential and admitted patients.

● Interviews incoming patient or representative and enters data into EMR; assures admission criteria is met .
