Responsible to act as a role model and ensure team compliance with customer service standards. Assist with providing support to Medical Assistants and LVN with outpatient care,medications and scheduling. Engage patient regarding healthcare education, medication administration, triage walk in as needed. Maintain medication cabinet is fully stocked along with expiration dates, participate in reporting and documenting accurately clinical finding and provide information to Providers. Responsible for answering phones, checking in patients with demographics, scheduling appointments, data entry, posting payments, preparing bank deposit slips, order inventory, help with medical records, scanning documents into patient chart, greeting and assisting patient sign in.

Responsible to assist with directing patient to appropriate level/location of care based on history, symptoms and Providers advice. Responsibilities include determination whether patient needs ER level of services of GMG services. Coordinates with POD's, Urgent care, Case Management and other GMG staff so that they can be informed and prepared for patients. Assist with home health agency, reports concerns, lab alerts, STAT and critical values reported. Document in a timely manner and send messages thru EMR, assist with medication refills, DX codes, and other services. Responsible to educate patients in relation to disease process, treatments, symptoms and provide orders and reassure and encourage patient of recommendation. Responsible to demonstrate excellent written skills and verbal.

Responsible for medication refills, faxes, and secure health messages requesting refills and prior authorizations. Responsible to confirm that pharmacy is the same as default pharmacy in prescription writer. Pharmacy may have changed and conduct outbound calls to patient to confirm preferred pharmacy. Responsible to confirm CMA protocol list and follow all protocol requirement and make sure that the patient meets criteria and at the time to refill or reject refills. Responsible to read/review all previous/present messages in patient chart and confirm lab criteria was met. Responsible to schedule appt through Televox. Team lead and assist with distribution process of SHM/faxes among 8 coworkers. I would attend weekly meeting to help with process of improvement within our department to better our patients and PCP and Department

Responsible for providing direct and indirect nursing care to patients in a Ambulatory setting in accordance with UHS policies and standards. Support and promote UHS values to ensure the patients/guest relations. Perform the Nursing process in a safe therapeutic manner in designated clinical settings. Maintain facilities and promotes department policies and standards. Initiate prior authorizations, assist Providers with thyroid biopsy, assist with Nursing visits for Specialty injections, assist with CGM downloads. Educate patient on how to use insulin pens and DM machines. Responsible to order all Endocrinology supplies and catering orders for meetings.

Responsible for direct/indirect patient care activities and perform clerical duties, medical records, support and maintain the UHS policies, values and guest relations. Triage patient with all vitals, injections, lab draw and provider patient with discharge summary instructions. Responsible for screening diabetic patients for DM Retinopathy by piloting program and taking photos of the eye and checking damage of retina.
