Additional Information
I have spent the last 24 years in health care system. I worked as a CNA/CMT in a Skilled Nursing from 1989 till 1995. I graduated from nursing school and started my practice in St Louis. During my clinical rotation in Public Health, I had an opportunity to work in Rock County Department of Health where we performed a lot of complex case management responsibilities such as assisting patient to get a better housing, follow up on well check up. These cases involved patient/family from all social-economic background. We developed plans for patient/family self-care competence including motivational assessment assessing for desired level of involvement and to adhere to care plan. I was also involved in assisting in placement for the patients post discharge.
