As a triage nurse with Gilchrist, I am responsible for triaging calls from patients and family members. I provide listening and support by phone and determine if further interventions are needed. I collaborate with case managers, physicians and field nurses to provide care for the termanilly ill. I meet patients and families where they are to assist when their regular and familiar team is not available. I act as a support for the field nurses and dispatch nurses when an in person visit is needed.
