I was an admission nurse for patients being discharged from the hospital. I developed plan of care and collaborated with MD to obtain orders. I assessed pt for other needs and obtained orders for PT/OT and Social works and provided and individualized plan of care for each patient. I provided education to patients and family for home infusion such as antibiotics , TPN and pain management using PCA pumps. Patients were also instructed on wound care at home to perform between skilled nurse visits.

I piloted the Home Med Monitor program. Patients were able to transmit vital signs to our office so nurses could provide guidance and education to prevent rehospitalizations.

I provided pre op and post op care for patients needing cardiovascular surgery . I cared for patients who had coronary bypass, aortic and mitral valve replacements , aortic aneurysms , thoracotomies and carotid endarterectomy. I provided education to patients and families on every aspect of care until patient was transferred to step down. I was a level 4 clinician on career ladder at Baptist, preceptor to new employees , member of unit steering committee, and was nominated for Shelby County top 100 nurses while working there. I was ACLS certified and able to assist with emergency situations.
