• Respite/Attendant Care/Habilitation

Respite care services are designed to provide relief for the primary caregiver. This service provides for short-term care and supervision in accordance with the person's Individual Support Plan. This service can be provided in the community, in the consumer's home or in the home of the DSP (if certified).

Attendant care is designed to ensure a level of self-reliance which has already been established. The focus of attendant care is to maintain a healthy and safe environment and respond to your personal care needs. This service is based on your specific needs, as identified in the Individual Support Plan and an Attendant Care Agreement.

Habilitation care enhances skills in the areas of self-care, home-living, community use, communication, social, health, safety, leisure, self-direction, and functional academics. Our DSP will listen to choices made by family member/guardian in determining what outcomes and objectives (goals) will meet expectations. Our DSP will utilize Individual Support Plan and to expressed expectations to develop habilitation objectives and outcomes directed at realizing individuals personal goals. These services can be provided in the community, individuals home and in the home of the DSP (if certified).

First point of contact for escalated request, Over see a team of 4-6 associates,Responsible for the Assignment of work to Associates, Monitor and/or complete production as applicable, Log and track errors, Go over audits, coaching associates as needed, Send out closed numbers, Act as a resource for Representatives questions and concerns, Act as a resource for upper management with process directions, Assists Supervisor/Manager with coaching, on-the-job training, monitoring work, serve as a back-up in production and works with other departments with escalated issue and resolve

Working with multiple health plans (Cigna, Beechstreet, Sloan’s, Blue cross & Cofinity) verifying patient medical coverage/eligibility, benefits and process authorization request for Home health care and Home medical equipment.
